CFMC Commits to Chronic Disease Management
Caswell Family Medical Center (CFMC) has been committed to providing access to health care for over 38 years. CFMC has been recognized as a state and national leader in terms of helping patients manage the effects of Diabetes.
CFMC is committed to helping patients manage their chronic conditions. Currently, 77.5% of CFMC’s diabetic patients are considered well controlled. The state and nation are catching up to us so those current numbers aren’t nearly as impressive!
“While we have a lot to be proud of, our community has a long way to go”, says William Crumpton, CFMC’s Chief Executive Officer.
More motivated than ever to see things change for the future of Caswell’s residents who are affected by diabetes, partnerships are forming.
CFMC is partnering with the Piedmont Triad Regional Council and Caswell Senior Services to offer access to “Diabetes Self Management Education”. This particular partnership provides education using evidence-based strategies over a six-week program to help patients who have uncontrolled diabetes. At the same time, CFMC is partnering with the Caswell County Health Department to provide patients who are at risk of developing diabetes with access to evidence-based educational programs to help prevent the onset of the disease. Between these two programs, CFMC has referred a total of 45 patients.
“Both of these programs are new to Caswell. As we work together to further develop the programs over time, we hope to offer these kinds of programs on an ongoing basis”, says Crumpton. “It’s about working together to bring opportunities to Caswell that can really help ‘move the dial’ in a positive direction.”
