Quit Smoking:
“Quitting is a journey”. Here you can find support, tips, tools, and expert advice to help you or someone you love quit smoking. You can even try a practice quit to help you quit for good!
Maintain a Healthy Weight (Adults):
Are you overweight, or maybe even considered “morbidly obese”? If so, you are among many who have attempted a diet and hit a plateau, only to get back to your weight, or even gain more.
To help you reach and maintain your desired weight, the National Heart, Lung, and Blood Institute has a few answers to some common questions and a few tools to help you along the way.
Maintain a Healthy Weight (Children):
Another great education tool is We Can! (WECAN), which stands for “Ways to Enhance Children’s Activity and Nutrition” and is a national science-based education program designed to give parents, caregivers, and entire communities the tips, tools, and strategies they need to help children maintain a healthy weight.
The program focuses on three vital behaviors in children:
- Improving food choices
- Increasing physical activity
- Reducing screen time (TV, tablets, phones, computers)
Healthy Eating:
The USDA’s “Choose My Plate” program helps you find your healthy eating style and build it throughout your life. It’s all about the right mix of foods and many factors, including your stage in life, your situations, access to food, culture, traditions, and of course, the personal decisions we make.
MyPlate will help guide you through your healthy eating style and has a great number of awesome ideas and instructions.
Diabetes:
The Robert Wood Johnson Foundation has a national program called the “Diabetes Initiative” that focuses on improving self-management for adults living with diabetes. Self-management programs are proven to improve diabetes care.
For those who live with diabetes, you know it’s a 24/7 job for the rest of your life. These educational tools will help you along the way with goal setting, healthy coping, healthy eating, and other self-management behavior tools.
Diabetes (Blood Glucose Log):
Is your diabetes at the point where you need to log your blood glucose? If so, don’t worry about having to create a log yourself. Here you will find a complete log template to record your daily blood glucose so you can bring it to your next appointment with your PCP.
The log holds more than a week’s worth of readings for you to share with your provider.
Hypertension (Facts):
Know the facts about High Blood Pressure! This patient education resource published by the CDC reviews the basics of high blood pressure in addition to a variety of available resources for patients to learn more about high blood pressure.
Hypertension in African-Americans:
This patient-friendly site is designed for African-American patients to educate themselves on heart health. It provides education specific to African-Americans to take control by understanding the risks and taking simple steps to address them.
The risk of heart disease and stroke are higher for African-Americans. Improve your odds of preventing and/or beating these diseases by understanding risks, then taking the steps to address them.
Hypertension (Blood Pressure Log):
This blood pressure log provides guidance on how to check your blood pressure. The log holds more than a month’s worth of readings for the patient to share with his or her healthcare provider.
The log has easy to follow instructions from the American Heart Association (AHA).
Hypertension and The Power to End Stroke:
Power to End Stroke is a campaign by the American Heart Association (AHA) designed to educate and bring awareness to reducing the incidence of stroke and to empower Americans to take charge of their health.
With this campaign, you can learn how to prevent, treat, and beat stroke.
Depression Toolkit:
The University of Michigan Depression Center has released a depression toolkit to help you take care of yourself.
Depression is a disease, but it is a manageable disease, like other chronic conditions like diabetes. While CHC offers outpatient psychiatric services and behavioral health services in Caswell County, this toolkit can help you manage your depression with self-care.
