Care Manager RN
Compassion Health Care is seeking a dynamic individual to work as a Care Manager and help establish our Care Management Department. The Care Manager needs to possess the characteristics of great work ethic, sound clinical judgment, team player, and demonstrates our core values of compassion for others, commitment, integrity, excellence, and equity in their daily work.
This position is responsible for managing at-risk, chronically ill patients to improve quality of care through the efficient use of resources, thereby enhancing the quality of care to achieve positive clinical outcomes by working with patients, families, providers, nursing staff, community partners, and insurance companies. The RN Care Manager is able to multitask and handle numerous responsibilities throughout each day. The Care Manager needs to be a highly effective communicator. The Care Manager will need to learn to understand and explain the features of Medicare, Medicaid, and other insurance options and have in-depth knowledge of health care services and medical terminology.
**The position will primarily work remotely but will need to have the ability to come to both health centers when needed (Yanceyville, NC and Eden, NC). **
Responsibilities:
- Assessing new patients for care,
- Demonstrates knowledge in medical care; such as diagnostic procedures, medication, symptoms, and other treatment-related therapies,
- Evaluates and assesses social determinants of health,
- Coordinating medical treatment, including administering assessments, developing, implementing and revising care plans to meet established patient and family goals, monitoring medication compliance,
- Promotes patient self-management and empowers families to achieve maximum levels of wellness and independence,
- Implements physician orders and nursing procedures,
- Educates patients and families about health care plans and options,
- Relationship building with patients, staff, and community including the Clinical Intergraded Network (CIN),
- Creates an environment of open communication with all parties,
- Reviews utilization and quality reports routinely and scans for gaps in care to identify patients needing the additional support of care management,
- Serves as advocates for and champions of the patient’s health,
- Connecting patients with social and community support systems for continuity of care,
- Ensures the patient’s chronic conditions are managed properly through coordination of care,
- Accurately documents patient care in the electronic health record and has proficient computer skills,
- Promotes collaborative teamwork and participates in departmental and organizational committees, as applicable,
- Participates in regular team meetings and peer review activities,
- Participates in the orientation of new personnel and precepts and mentors peers,
- Ability to understand and complete basic financial statements for the purposes of budgeting department needs and determining growth capabilities with assistance of supervisor and financial department.
Job Qualifications:
Associate or Bachelor’s degree from an accredited School of Nursing. Previous clinical experience in Care Management (3-5 years), and experience in a rural health care setting is preferred. Current/valid NC Nurse’s license required. Basic Cardiac Life Support (BCLS) is required. Advanced Cardiac Life Support and Pediatric Advanced Life Support (PALS) are preferred. Experience with Electronic Medical Records required.
