Clinical Operations Manager Receives Case Manager Certification

Patricia (Tricia) Towler, CFMC’s Clinical Operations Manager, has recently received her Case Manager Certification by The Commission for Case Manager Certification (CCMC). CCMC is the first and largest nationally accredited organization that certifies case managers. This is a huge step for CFMC and Tricia.

According to CCMC, the definition of Case Management is:

A collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the client’s health and human service needs. It is characterized by advocacy, communication, and resource management and promotes quality and cost-effective interventions and outcomes.

With this certification, CFMC will continue to “provide client wellness and autonomy through advocacy, communication, education, and the identification and facilitation of services.”

As a patient, you want to be as healthy as you possibly can. CFMC helps you reach your health goals. As a patient-centered medical home, this certification allows us to go even further into providing you and your support system with a high level of wellness, self-management, and functional capability.

It is important to be there for our patients and community in a time of need. As a case manager, Tricia will assist patients in finding available resources that are cost-effective, efficient, and timely. With her case management certification, she possesses the education, skills, knowledge, and experience needed to provide necessary services to you.
