Position Overview

Compassion Health Care is seeking a dedicated and adaptable Clinical Care Coordinator (LPN, CMA) – Floater to support our clinical teams across multiple service lines, including Primary Care, Psychiatry, Urgent Care, and our School-Based Health Center. This role provides coverage at Caswell Family Medical Center (CFMC), James Austin Health Center (JAHC), and the Mobile School-Based Health Center (RV) as needed.

This position is ideal for a clinically skilled professional who values teamwork, flexibility, and patient-centered care while supporting multiple care environments.

Key Responsibilities

• Assist providers with patient care needs using sound clinical judgment
• Support referral and medication refill workflows
• Manage clinical documentation within the electronic medical record (EMR)
• Provide coverage when primary Clinical Care Coordinators are unavailable
• Utilize the nursing process (assessment, planning, implementation, evaluation, and documentation) to support patient care
• Administer medications and assist with medical procedures per provider orders
• Identify and address patient and family learning needs
• Support efficient clinic flow by anticipating provider needs
• Perform referral tracking and follow-up for completed and pending referrals
• Capture and document Chronic Care Management (CCM) time as appropriate
• Support coverage across all clinic sites and operate the Mobile School-Based Health Center RV (training provided)
• Maintain professional, cooperative relationships with providers and staff

Qualifications

• Licensed Practical Nurse (LPN) or Certified Medical Assistant (CMA)
• Active license or certification in good standing
• Strong clinical judgment and critical thinking skills
• Ability to work across multiple service lines and locations
• Willingness to travel between clinic sites and operate the mobile unit
